Abstract text: Surgery is a fundamental step in mammary cancer treatment, alongside chemotherapy, radiation and hormonal therapy. Radical operation is indicated in degree I and II, and also in degree III converted with neo-adjuvant chemo- and radiotherapy, after the size of the primary tumor was reduced. In these situations, nowadays is no longer indicated the Halstedt procedure, most used technique is Madden and rarely Patey operation. Alongside these, sectorectomy and quadrantectomy with axillary lymphadenectomy and radiotherapy are especially useful in little tumors. If the cancer has penetrated the muscular chest wall, Halstedt procedure is elected and the dissection must not injure the subjacent layers (pleura). Also, so-called „cleaning” mastectomy may be indicated in selected cases of advanced breast cancer, with skin invasion and putrid ulceration. In these patients there are supplementary difficulties consisting in hemorrhage caused by neo-angiogenesis, and the closure of remaining skin after tumor excision. We studied a lot of 312 women with breast tumors which were performed radical mastectomy or „cleaning” mastectomy. Radical mastectomy remains one of the primary option of breast cancer cure, alongside mammary sectorectomy with axillary lymphadenectomy and radiotherapy. However, in advanced cases of breast cancer with necrotic skin invasions, „cleaning” mastectomy can be used in order to improve patient’s life quality.
