Abstract text: Laparoscopic cholecystectomy has become the elective surgical treatment in lithiasis cholecystitis, substituting, in most cases, the open procedure. Classic version remains necessary in cases with complications in which the laparoscopical resolve is insecure. However surgeon must master both interventional procedures. The choosing of the path of approach depends, first, by the particularity of case, but, also measurably by surgeon experience. There are studied a number of 4909 cholecystectomies made during 13 years in the Municipal Hospital Surgery Clinic Arad. Of these 3498, representing 71.25% were performed laparoscopically and 1411, representing 28.75% were achieved by open procedure. Regarding the influence of laparoscopic surgery clinic experience, generally speaking, we observed that the percentage of laparoscopic cholecystectomy increased quickly from 48% in 1995, which is the starting year, to 54% in 1996 and 68% in 1997, following values of 80% in 1998 and still oscillating between 70 and 80% in the next 9 years (1999-2007). This stabilization reflects the maturation of laparoscopic surgical experience of the whole clinic. There are discussed issues of individual operating experience, remarking the rise of laparoscopic procedure percentage with the surgical experience. The conversion of laparoscopic cholecystectomy in open procedure must be made whenever the safety of operation requires, without regarding to considerations of pride and without delay.
