Abstract text: Introduction: 2009 AH1N1 pandemic flu affected mainly subjects less than 60 years contrary to seasonal flu which affect more subjects over 60 years-elderly patients are partly protected due the anterior immunization with similar sample. Results: There were 107 cases confirmed with pandemic H1N1 by RT-PCR, mean age 39 years; 51% were female. The onset symptoms were: fever over 38°C, myalgia and cough. A percentage of 4.67% admitted to ICU for SDRA and mechanical ventilation, with high case fatality rate-1 pregnant woman survived after cesarean section. These ICU cases were malignant, hypertoxic forms of influenza, no important comorbidities associated , laboratory indicators: relative lymphopenia, elevated serum transaminases; chest x-ray with bronchopneumonia and pulmonary edema; necropsy examination – reveals hemorrhagic bronchopneumonia, cerebral edema and pericarditis serouses and 1 hemorrhagic. A percentage of 2.8% patients with pneumonia and hypoxemia don’t require mechanical ventilation. 15% of patients had comorbidity: obesity, diabetes. Conclusions: Majority of cases not developed ARDS/MODS; was middle aged adults, no comorbidities. ICU patients with lung protective ventilatory strategy were adults without underlying comorbidities, with high case fatality rate.
