Abstract text: Gastric cancer is the second worldwide cause of cancer mortality. Most of the patients are diagnosed in advanced stages, namely IIIrd and IVth stage, and cannot benefit of surgical radical treatment. Thus stomach malignant tumor prevention is very important. There are many recent studies suggesting that gastric cancer is preceded by specific lesions that can be reversed by changing alimentary habits if they are diagnosed in low grade stages. The most important precancerous lesion of gastric mucosa is represented by gastric dysplasia. The diagnosis of premalignant lesions can be achieved only by histopathological examination of gastric lesions fragments. Of all dysplasia types, the severe form (high grade dysplasia) has the highest cancerous potential and is included in the same group with “in situ” carcinoma in the Vienna Classification. Detection of dysplasia have key role in the prevention of gastric cancer. It was studied a series of patients with endoscopic biopsies, following the results of histopathological exam. There are difficulties in distinguishing simple dysplasia from regeneration’s areas of gastric mucosa damaged. It is also very problematic the delimitation between severe dysplasia and “in situ” gastric cancer. Thus, these two pathological entities must be treated by endoscopic or surgical excision.
