Abstract text: Introduction: Liver hydatid cyst is a condition frequently diagnosed in Romania, which is a characteristic endemic area, and it is considered the most important zoonotic disease in Romania. Most serious complications are the hydatid cyst rupture into the peritoneal cavity, which is associated with intense abdominal pain or more serious anaphylactic shock. Case report: We present a case of a male patient, 56 years old, hospitalized for chest pains, arthralgia, seizure, subfebrility occurring for about one month before admission. Personal pathologic history of the patient reported the presence of bilateral ureteral stents, deployed for bilateral renal lithiasis complicated with bilateral hydronephrosis and fever. On admission, abdominal ultrasonography revealed an inhomogeneous tumour with well-defined limits, 6.9 cm in diameter situated in the left hepatic lobe, III/IV liver segment, which on Doppler ultrasound examination was avascular, and a disruption in the super-external region, hyperechogenic non transonic. Markers of the infection with genus Echinococcus were positive (antibodies anti-Echinococcus IgG type were significantly positive in the absence of antibodies anti-Echinococcus IgM type) and the CT examination confirmed the diagnosis of complicated liver hydatid cyst with surgical recommendation. Particularity: Presence of intrahepatic hydatid cyst complicated with infection and spontaneous rupture without septic distance determinations from a patient with immune deficiency, denutrited, with bilateral ureteral stents two months before admission for bilateral hydronephrosis.
