Abstract text: Our study purpose is to estimate the efficiency and safety of levonorgestrel Mirena implant versus hysterectomy in dysfunctional uterine bleeding treatment. Levonorgestrel (or l-norgestrel or D-norgestrel) is a synthetic progestogen used as an active ingredient in some hormonal contraceptives. Dysfunctional uterine bleeding is irregular bleeding from the uterus. In most cases, this problem is related to changes in hormone levels. Material and method – we studied a total of 146 women with hysterectomy indication for uterine bleeding, between August 2003 and March 2006. They were grouped in three lots: lot number one ( A ) – 81 patients with hysterectomy, lot number two (B) -35 patients with levonorgestrel Mirena implant insertion, and lot number three ( C ) – 30 patients with transcervical endometrial resection. From these the lot B and C are considered as conservative therapy. We observed the catamenial characteristics and other clinical symptoms, including the patients’ satisfaction after 1, 6 and 12 months. Results, discussions and conclusions: 1. Mirena system is safe and represents a real alternative to surgery in dysfunctional uterine bleeding treatment; 2. Mirena levonorgestrel implant improves the catamenial characteristics after 2-4 months and reduces the uterus and miomas size; 3. Mirena is a useful therapeutic alternative to hysterectomy in this pathology, maintaining also the reproductive function.
