Abstract text: We present a case of a 32 year old woman with cardiac arrest and chest pain that starts at 4 a.m. At 4.30 the physician from Ambulance Service made an ECG and the patient is without pain. After 10 minutes the pain starts again and the physician repeats the ECG. This time appear the signs of an acute myocardial infarction. The patient was transported to the Emergency Room. We start to apply the protocol for acute myocardial infarction. After 5 minute, when she is monitoring, present a ventricular fibrillation. We applied the protocol of Advanced Cardiac Life Support with biphasic defibrillation 200 joule, 200 joule, 200 joule, 200 joule with 1 mg Adrenaline intravenous after the second shock and 300 mg Amiodarone after the third shock, thoracic compression and ventilation with bag mask. After the 4th sock the patient become responsive and the rhythm become sinusal. After other 5 minute become another episode of ventricular fibrillation. We applied 200 joule, 200 joule and the patient become responsive and the rhythm become sinusal. The decision after the second episode of ventricular fibrillation was reperfusion therapy: thrombolysis with Metalyse. The patient became stable and didn’t repeat the cardiac arrest. Conclusion: Early defibrillation associated with early reperfusion therapy is the key treatment for cardiac arrest with ventricular fibrillation on a patient with acute myocardial infarction.
