Abstract text: | Introduction: Diplopia (double vision) is often the first manifestation of many systemic disorders, especially muscular or neurologic processes Diplopia is a common subjective complaint, or diplopia may be elicited during the course of an eye examination. A clear and comprehensive history is the single most useful evaluation in treating patients with diplopia. Lambert-Eaton myasthenic syndrome (LEMS) is a rare condition in which weakness results from an abnormality of acetylcholine release at the neuromuscular junction and one of the most frequent manifestations is diplopia especially after extensive work. Cancer is present when the weakness begins or is later found in 40% of patients with LEMS. Purpose: We report a case of small cell lung cancer (SCLC) associated with Lambert-Eaton myasthenic syndrome. The particularity consists in the lack of symptoms, diplopia and fatigability. Materials and methods: We present a Case report of a 52 year’s old female, non-smoker patient with Eaton-Lambert myasthenic syndrome as part of the paraneoplastic syndrome associated with SCLC. The patient complained of recurrent time periods of diplopia and fatigue that are present for 3-4 years. No other symptoms were reported. Chest X-ray revealed an abnormal shadow, and small cell lung cancer was diagnosed on the basis of biopsy specimens. Anti-voltage-gated calcium channel antibody was positive. Electrophysiological studies confirmed the diagnosis of Lambert-Eaton myasthenic syndrome. At the time of writing of this report the patient currently undergoes chemotherapy. Conclusion: Binocular diplopia can be the only eye related paraneoplastic symptom as part of LEMS in association with SCLC therefore it must be closely investigated. |
